Band Registration Forms

 

The following forms need to be downloaded and brought to band registration on Friday.  Time is 3:30-7:00.  

 

1. Band Handbook (7 total forms)

2. Criminal Background (FISD online form - parent information/not student)

3. Drug Testing 

4. 8-hour rule acknowledgment form

5. Media release

6. Code of conduct

 

You should bring 12 forms with you to registration.
